I can recommend these guys:


They can ship rebuilt actuators worldwide (bearing in mind that they need your old units in exchange), or if you're in the UK they offer a mobile fitting service.

I used the mobile service - the swap was performed efficiently and, when one of the new units proved faulty, they came back and swapped it again.

In particular it's worth noting that they don't just replace the gears, they also look at the electronics - my problems had been manifesting for months as what looked like a dying battery (even though it was brand new), before the actuator fault codes actually become consistent enough to make the change. I don't think replacement gears alone would have done any good for me.
