I am thinking of getting 3.45 gears installed but would like to hear some real world numbers on gas mileage.

Assuming you don't drive it harder, the only difference should be cruising. Until cruising, you are simply in a higher gear sooner. I would estimate 10%, about 2mpg. Others will chime in with experience.

i never knew the M5 was purchased for its fuel efficiency!

I have the 345's, my obc says i average 14.7mpg. Most of my driving is city and 2 lane rural types of roads (fun driving, which likely accounts for the mpg). I've seen 26 on a highway run, really trying to get it, but general hwy mpg seems to be around 21 or so. Honestly, it didnt make any difference according to my car's obc.

I don't drive it any harder than before I made all of the Dinan mods and I still get 16 around town and 20-21 on the highway.
I would give up some fuel economy for the seat of the pants rush.
