Coolant leak on the back of the head

I replaced my spark plugs and while I had the tools out I decided to re-install the two brackets that are removed to get to the Intake CPS. I had swapped CPS a few weeks ago and not put the brackets back on because they are something of a PITA to get to. After completing everything I start it to make sure it runs and shut it off and jack up one side to to put the under body covers on. This is when I notice a pool of coolant on the ground. Long story short, I must have mixed one of the bolts for the brackets with the ones for the coil ground lead. That one bolt was a touch too long. The bolts for these brackets screw into two ears on a plate that seals a coolant passage on the back of the head. See parts 20 and 21 in the diagram attached. It is very hard to see anything back there, but it appears that when the bolt bottomed out on the head, it jacked the plate out and may have cracked or bent it such that it now leaks. With all three bolts on the plate properly torqued, it still seaps and I think I can see a crack in it.

Has anyone replaced this plate and o-ring while the head was on the car? My concern is it will come off easily, but getting the new o-ring to stay in place looks like it could be a challenge. I would appreciate input from anyone who has experience with this plate and o-ring.
