Sending ECU to get flashed, a few questions.

I will be sending my ECU to D/A to get it flashed but have some practical questions for those who have done it:

1) What precautions should I take when packaging it up?

2) How much do I ensure it for, in case it never shows up or shows up damaged? Without that ECU, I am $%@%^ .

3) Should I send it USPS, UPS or Fedex (leaning towards UPS)

4) Any tips on how to remove it? Found one thread on board about how to do this.
